Do you live with a long term condition affecting your health and wellbeing?

In the Aylesbury Vale area, there are over 60,000 people living with a long term health condition, such as diabetes, arthritis or asthma. Living with any health condition can impact on all aspects of your life, and the support you need may vary over time.

The Aylesbury Vale Clinical Commissioning Group (CCG) is keen to gain a better understanding of patient needs in relation to long term conditions, so it can assist local GPs. In particular, they want to get better at:

  • responding better to each patient's individual needs
  • preventing disease, not just treating it
  • giving patients more control over their own health

Aylesbury Vale CCG is therefore inviting individuals with long term conditions to share their story at an event taking place on Tuesday 25th March, from 5:30pm-7:30pm at The Gateway building (the AVDC Headquarters) in Aylesbury. The Gateway is located on Gatehouse Road (HP19 8FF) near the A41 Bicester Road roundabout, and opposite the former Focus DIY store (now cleared and earmarked for a new Sainsbury's store). The event will be written up and will be used to inform a wider consultation, and ultimately, to help the Aylesbury Vale CCG build better systems to help you live your life.

About the event
This is about you sharing your experience with the CCG. In a relaxed environment, in small groups, with the help of a facilitator, CCG staff will explore the experiences of the group. In particular they would like to understand what support you needed at different stages and how the CCG could develop more resources to help people to self-care.

Aylesbury Vale Clinical Commissioning Group (AVCCG) is the name for a group of twenty-one GP practices that provide first line (primary) medical services in Princes Risborough, Thame, Haddenham, Aylesbury, Buckingham and across North Buckinghamshire. You can find out more here.
